What is Bazedoxifene
Bazedoxifene is a third generation selective estrogen receptor modulator (SERM), developed by Pfizer following the completion of their takeover of Wyeth Pharmaceuticals. In late 2013, Pfizer received approval for bazedoxifene as part of the combination drug DUAVEE in the prevention (not treatment) of postmenopausal osteoporosis. It is approved in the European Union (marketed in Italy and Spain) and Japan, and is in the late phases of review by the United States’ Food and Drug Administration (FDA). Bazedoxifene is yet to be approved by the FDA.
Bazedoxifene mechanism of action
Bazedoxifene belongs to a class of compounds known as selective estrogen receptor modulators (SERMs). Bazedoxifene acts as both an oestrogen-receptor agonist and/or antagonist, depending upon the cell and tissue type and target genes. Bazedoxifene decreases bone resorption and reduces biochemical markers of bone turnover to the premenopausal range. These effects on bone remodelling lead to an increase in bone mineral density (BMD), which in turn contributes to a reduction in the risk of fractures. Bazedoxifene functions primarily as an oestrogen-receptor antagonist in uterine and breast tissues.

Toxicity of Bazedoxifene
In all clinical trials bazedoxifene, given either alone or in combination with CE appeared to be well tolerated, with few patients experiencing side effects. Phase I studies of bazedoxifene alone demonstrated that the compound is safe, with no toxicity at doses up to 80 mg, significantly higher than those currently used in ongoing phase II/III studies

What is Duavee?
Duavee contains a compound of bazadoksifena and conjugated estrogens. Bazadoksifen blocks estrogen from certain cells in the lining of the uterus to protect against excessive growth of uterine tissue. Estrogen is a female sex hormone produced by the ovaries. Conjugated estrogens are a mixture of estrogen hormones.
Duavee is used briefly in women after menopause to treat moderate to strong hot flashes and to prevent osteoporosis. This medicine is used in women who have not had a hysterectomy (uterus removed). Conjugated estrogens should not be used to prevent heart disease, stroke, or dementia, as this medicine may actually increase the risk of developing these conditions.
Do not take other estrogens while you are taking Duavee. You should not use this medicine if you have one of the following conditions: a history of heart attack, stroke, or blood clot; liver disease; Abnormal vaginal bleeding; A hormone-related cancer, such as breast or uterus cancer.
Duavee may increase the risk of developing breast or uterus cancer, and may also increase the risk of dementia, heart attack, stroke, or blood clot. Ask your doctor about individual risk. Duavee is intended for short-term use at the lowest dose needed to treat your condition.
