What is Cefepime
Cefepime is a fourth-generation cephalosporin antibiotic developed in 1994. Cefepime has an extended spectrum of activity against Gram-positive and Gram-negative bacteria, with greater activity against both Gram-negative and Gram-positive organisms than third-generation agents. Cefepime is usually reserved to treat severe nosocomial pneumonia, infections caused by multi-resistant microorganisms (e.g. Pseudomonas aeruginosa) and empirical treatment of febrile neutropenia.
Cefepime mechanism of action
Cephalosporins are bactericidal and have the same mode of action as other beta-lactam antibiotics (such as penicillins). Cephalosporins disrupt the synthesis of the peptidoglycan layer of bacterial cell walls. The peptidoglycan layer is important for cell wall structural integrity, especially in Gram-positive organisms. The final transpeptidation step in the synthesis of the peptidoglycan is facilitated by transpeptidases known as penicillin binding proteins (PBPs).

Indication of Cefepime
For the treatment of pneumonia (moderate to severe) caused by iStreptococcus pneumoniae/i, including cases associated with concurrent bacteremia, iPseudomonas aeruginosa/i, iKlebsiella pneumoniae/i, or iEnterobacter/i species. Also for empiric treatment of febrile neutropenic patients and uncomplicated and complicated urinary tract infections (including pyelonephritis) caused by iEscherichia coli/i or iKlebsiella pneumoniae/i, when the infection is severe, or caused by iEscherichia coli/i, iKlebsiella pneumoniae/i, or iProteus mirabilis/i, when the infection is mild to moderate, including cases associated with concurrent bacteremia with these microorganisms. Also for the treatment of uncomplicated skin and skin structure infections caused by iStaphylococcus aureus/i (methicillin-susceptible strains only) or iStreptococcus pyogenes/i and complicated intra-abdominal infections (used in combination with metronidazole) caused by iEscherichia coli/i, viridans group streptococci, iPseudomonas aeruginosa/i, iKlebsiella pneumoniae/i, iEnterobacter/i species, or iBacteroides fragilis/i.
Toxicity of Cefepime
Symptoms of overdose include seizures, encephalopathy, and neuromuscular excitability.
