Laboratory in vitro plasma protein binding studies using equilibrium gel filtration have shown that 99.5% of pimecrolimus in plasma is bound to proteins over the pimecrolimus concentration range of 2-100 ng/mL tested. The major fraction of pimecrolimus in plasma appears to be bound to various lipoproteins. As with other topical calcineurin inhibitors, it is not known whether pimecrolimus is absorbed into cutaneous lymphatic vessels or in regional lymph nodes.

What is Pimecrolimus
Pimecrolimus is an immunomodulating agent used in the treatment of atopic dermatitis (eczema). It is currently available as a topical cream, once marketed by Novartis, (however Galderma will be promoting the molecule in Canada in early 2007) under the trade name Elidel. (Wikipedia)
Pimecrolimus mechanism of action
Pimecrolimus binds with high affinity to macrophilin-12 (FKBP-12) and inhibits the calcium-dependent phosphatase, calcineurin. As a consequence, it inhibits T cell activation by blocking the transcription of early cytokines. In particular, pimecrolimus inhibits at nanomolar concentrations Interleukin-2 and interferon gamma (Th1-type) and Interleukin-4 and Interleukin-10 (Th2-type) cytokine synthesis in human T cells. Also, pimecrolimus prevents the release of inflammatory cytokines and mediators from mast cells in vitro after stimulation by antigen/lgE.

What is Elidel?
Elidel is an immunosuppressant. It works by reducing the body’s immune system to slow the growth of atopic dermatitis (eczema) on your skin. Elidel Cream is used to treat severe atopic dermatitis when other drugs do not work well.
You should not use Elidel if you are allergic to pimecrolimus, or if you have a bacterial or viral skin infection (including herpes or chicken pox).
Before using Elidel Cream, tell your doctor if you have skin cancer or any type of skin infection, Netherton syndrome, a weak immune system, kidney disease or swelling, redness or irritation on large areas of the skin. Avoid using Elidel Cream in an area that may be affected by skin cancer.
Elidel can lower blood cells that help your body fight infections. This can ease your illness from the virus. Tell your doctor if you have had any illness, or if you have any signs of infection, such as fever, chills, sore throat, or flu symptoms.
