Tiotropium is a muscarinic receptor antagonist, on topical application it acts mainly on M3 muscarinic receptors located in the airways to produce smooth muscle relaxation, thus producing a bronchodilatory effect.

Toxicity of Tiotropium
No mortality was observed at inhalation tiotropium doses up to 32.4 mg/kg in mice, 267.7 mg/kg in rats, and 0.6 mg/kg in dogs. These doses correspond to 7,300, 120,000, and 850 times the recommended human daily dose on a mg/m2 basis, respectively.

What is Spiriva?
Siriva contains tiotropium bromide – a substance with a prolonged m-anticholinergic action. It has an affinity for M1-M5 muscarinic receptors. As a result of receptor inhibition, relaxation of the smooth muscles of the respiratory tract is observed. Bronchodilatory effect dose-dependent, long-lasting (about a day).
During inhalation, a selective action in the form of bronchodilation is noted, systemic reactions are rarely observed. The drug increases FEV1, FZHEL. Cases of development of tolerance were not observed. The use of the drug Spiriva has a positive effect on the quality of life of patients with COPD, reduces the number of hospitalizations.
